• In cases of sudden illness, call our medical office. We will fit you into our daily schedule as soon as possible. It may be necessary for a physician other than your own to examine you.
• To reach the physician on call after office hours, call (317) 631-3466.
• If you experience a medical emergency, call 911 or go to the nearest hospital emergency room  and ask the staff to contact your physician.

  • Medical condition(s)
• For prescription refills, call (317) 786-9285 during business hours. Your prescription will be called into your pharmacy within the next 24 hours.
• Co-payments are expected at the time of service. If you have questions regarding your statement, please call our billing office at (317) 780-3333. Our billing office can assist in arranging a payment plan, if necessary.
