We work hard to provide a high standard of care and treatment and are committed to putting the needs of the patient first. Unfortunately, sometimes things don’t go as expected. If this happens, we want to know so that we can ensure the same thing doesn’t happen again.
What should I do if I have concerns about my care or your services?
If you have a concern about your care, the care of a relative or about any aspect of our services, please talk to a member of staff who is with you at the time. They may be able to resolve your concerns right away.
If you are unable to resolve your concerns by talking to a member of staff, contact the Service Excellence department. Please be assured your care will not be affected as a result of making a complaint. You can contact Service Excellence at (317) 782-6000 or complete the form below.
What should I do if I want to make a written complaint?
We value your feedback and take all complaints seriously. Your comments will help us improve care for our patients. If you wish to make a formal complaint you can do so by completing the form below or by writing to the Service Excellence department at the following address:

What happens next?
Your complaint will be acknowledged in writing within seven business days.
If you are complaining on behalf of someone else, we may first need to obtain written permission from that person due to federal privacy guidelines (HIPAA).
We will notify the appropriate administrative staff, you will be contacted to discuss your concerns, and upon conclusion of our investigation, you will receive a written response to confirm the action we have taken as a result of your concerns.
