Birth and Death Certificates

St. Francis Hospital & Health Centers does not issue birth or death certificates. These come through the State of Indiana.

Certified copies of birth and death certificates are available from the Indiana State Department of Health (ISDH) or from the local health department in the county where the event occurred. All requests require proper identification. Orders may be placed in person or by mail. Proper identification, such as a driver's license or passport, is required at the time of the request. Online ordering is available with an additional processing fee.
